BY: M. of A. Simpson

COMMEMORATING the 40th Anniversary of the Hudson
        Headwaters Health Network

WHEREAS, It is the sense of this Legislative Body to commend those
organizations that work to serve the health care needs of citizens of
the State of New York; and

WHEREAS, Attendant to such concern, and in full accord with its
long-standing traditions, this Legislative Body is justly proud to
commemorate the 40th Anniversary of the Hudson Headwaters Health
Network; and

WHEREAS, The theme for this milestone anniversary, FortyForward, is
all about reflecting on the past while focusing on the future as this
vital organization envisions the next forty years of accessible health
care; most recently, the first Hudson Headwaters Mobile Health center
hit the road, providing care for those who are underserved; and

WHEREAS, For four decades, this innovative, sustainable and
community-focused health system has stayed true to its mission of
providing the best health care, and access to that care, for everyone in
its communities through comprehensive primary care and diverse
partnerships; and

WHEREAS, A Federally Qualified Health Center (FQHC), Hudson
Headwaters Health Network provides care to everyone in the communities
it serves, regardless of income or insurance; and

WHEREAS, From its humble beginnings of a single health center in
Chestertown, Hudson Headwaters Health Network has grown to a network of
21 community health centers providing primary care to more than 7,200
square miles of the Adirondack North Country and Glens Falls region; the
network serves all of Warren County and parts of Clinton, Essex,
Franklin, Hamilton, Saratoga and Washington counties; and

WHEREAS, Hudson Headwaters Health Network is the sole medical
provider in much of this rural, medically underserved region, and in the
Glens  Falls  area,  it  is the only primary care "safety-net" provider;
their patients are served in Hudson Headwaters' local health centers, in
hospitals, nursing homes, work places, schools and in their homes; and

WHEREAS, In 2020, Hudson Headwaters Health Network's practitioners
cared for more than 1,200 people a day, accounting for about 425,000
patient visits; and

WHEREAS, The success of the Hudson Headwaters Health Network is in
direct correlation to the efforts of its CEO Dr. Tucker Slingerland, its
physicians, nurses and expert staff, whose involvement is, and always
has been, characterized by an impressive commitment, an unbridled
enthusiasm, and an uncompromising standard of excellence in all
endeavors on behalf of the institution and the community it serves; and

WHEREAS, This Legislative Body recognizes the vital importance of
the Hudson Headwaters Health Network in providing excellence in care
through prevention, education and treatment, and its evident commitment
to this mission; now, therefore, be it

RESOLVED, That this Legislative Body pause in its deliberations to
commemorate the 40th Anniversary of the Hudson Headwaters Health
Network; and be it further

RESOLVED, That a copy of this Resolution, suitably engrossed, be
transmitted to Dr. Tucker Slingerland, CEO, Hudson Headwaters Health
Network.
