        AN ACT to amend the social services law, in relation  to  directing  the
          commissioner  of  health to develop a standard prior prescription drug
          authorization request form for managed care providers
 

          The People of the State of New York, represented in Senate and  Assem-
        bly, do enact as follows:
 
     1    Section  1.  Section  364-j  of  the social services law is amended by
     2  adding a new subdivision 26 to read as follows:
     3    26. The commissioner of health shall develop a standard prior authori-
     4  zation request form or forms to be utilized by all managed care  provid-
     5  ers for purposes of submitting a request for a utilization review deter-
     6  mination  for  coverage  of prescription drug benefits under this title.
     7  The managed care provider shall make the  standard  prior  authorization
     8  request  form  or  forms available to, and shall accept it or them from,
     9  prescribing providers in paper and electronic form.

    10    § 2. This act shall take effect immediately; provided,  however,  that
    11  the  amendment  to  section  364-j  of  the social services law, made by
    12  section one of this act shall not affect the repeal of such section  and
    13  shall be deemed repealed therewith.
