  TITLE OF BILL: An act to amend the public health law, in relation to requiring physi- cians and hospitals to obtain the name of the school attended by school- aged patients and to include this information in their admission regis- tration forms   PURPOSE OR GENERAL IDEA OF BILL: An act to amend the public health law, in relation to requiring physi- cians and hospitals to obtain the name of the school attended by school- aged patients and to include this information in their admission regis- tration forms,   SUMMARY OF PROVISIONS: § 230-f. Registration requirements. Every physician shall inquire and document as part of his or her patient registration, the name and address of the school attended by such physician's school-aged patients and include such information in the patients' medical registration form, § _2805-aa. Admission registration requirements.         AN ACT to amend the public health law, in relation to  requiring  physi-
          cians  and  hospitals  to  obtain  the  name of the school attended by
          school-aged patients and to include this information in  their  admis-
          sion registration forms

          The  People of the State of New York, represented in Senate and Assem-
        bly, do enact as follows:
 
     1    Section 1.  The public health law is amended by adding a  new  section
     2  230-f to read as follows:
     3    §  230-f. Registration requirements. Every physician shall inquire and
     4  document as part of such physician's patient registration, the name  and
     5  address  of the school attended by such physician's school-aged patients
     6  and include such information in the patients' medical registration form.
     7    § 2. The public health law is amended by adding a new section  2805-aa
     8  to read as follows:
     9    §  2805-aa. Admission registration requirements. All general hospitals
    10  shall inquire and document the name and address of the  school  attended
    11  by  school-aged patients upon admission, and include such information on
    12  the hospitals' admission registration form.
    13    § 3. This act shall take effect one year after the date upon which  it
