TITLE OF BILL:
An act to amend the elder law, in relation to the dissemination of
information pertaining to Alzheimer's disease and other forms of demen-
tia through the NY Connects: Choices for Long Term Care program
 
PURPOSE OR GENERAL IDEA OF BILL:
This bill would encourage the sharing of information about programs and
services that treat and support individuals with Alzheimer's disease or
other forms of dementia.
 
SUMMARY OF SPECIFIC PROVISIONS:
Section 1. This section would require NY Connects: Choices for Long Term
Care program to include programs and services for individuals suffering
Alzheimer's disease or other forms of dementia.
Section 2. Effective Date.
 
JUSTIFICATION:
There are many programs across the state that are designed to assist and
support individuals with Alzheimer's disease and dementia. Innovative
ideas are implemented at the local level, but it is difficult for groups
in other areas of the state to find out about them. Information about
these innovative and successful programs should be available through the
NY Connects program so that their success can be replicated in other
areas.

EFFECTIVE DATE:
This act shall take effect one year after it shall have become a law.
Effective immediately, the addition, amendment and/or repeal of any rule
or regulation necessary for the implementation of this act on its effec-
tive date is authorized to be made and completed on or before such
effective date.

AN ACT to amend the elder law, in relation to the dissemination of
information pertaining to Alzheimer's disease and other forms of
dementia through the NY Connects: Choices for Long Term Care program
The People of the State of New York, represented in Senate and Assem-bly, do enact as follows:
1 Section 1. Subdivision 8 of section 203 of the elder law, as added by
2 section 18 of part B of chapter 58 of the laws of 2007, is amended to
3 read as follows:
4 8. The director, in consultation with the commissioner of health,
5 shall establish a program to be known as the NY Connects: Choices for
6 Long Term Care. The purpose of this initiative is to provide consistent,
7 comprehensive, locally-based information and assistance on long term
8 care services to consumers, caregivers and families to help them make
9 educated choices. This program shall provide individuals, caregivers,
10 and families with objective information and assistance about home,
11 community-based and institutional long term care services, including
12 programs and services for individuals suffering from Alzheimer's disease
13 or other forms of dementia. NY Connects [will] shall be available on a
14 voluntary basis to consumers, caregivers and their families. There shall
15 be an on-going education and outreach campaign to educate the public
16 about long term care services and programs and services for individuals
17 suffering from Alzheimer's disease or other forms of dementia available
18 in their community and to assist consumers in preparing for their long
19 term care needs.
20 § 2. This act shall take effect one year after it shall have become a

22 any rule or regulation necessary for the implementation of this act on
23 its effective date are authorized to be made and completed on or before
24 such effective date.
