Relates to the heroin and opioid addiction wraparound services demonstration program; empowers the commissioner to develop a heroin and opioid addiction wraparound services demonstration program for adolescents and adults who have entered and successfully completed treatment programs; provides that the program shall remain available to these individuals at least nine months after the completion of such program; identifies five relevant jurisdictions, as well as a best-practice model to deliver relevant services, and the commissioner shall provide a written evaluation of the demonstration program. |
