AN ACT to amend the insurance law and the public health law, in relation
to the use of insurance identification cards
The People of the State of New York, represented in Senate and Assem-bly, do enact as follows:
1 Section 1. Section 3217-a of the insurance law is amended by adding a
2 new subsection (f) to read as follows:
3 (f) Each insurer subject to this article shall issue to each insured
4 an insurance identification card which shall include the following
5 information:
6 (1) the insured's identification information, including the insured's
7 name and identification number;
8 (2) the name of the product the insurer uses to describe the benefits
9 covered under the contract;
10 (3) the amount of the insured's co-payment, if any; and
11 (4) the billing address and contact information, including a telephone
12 number, for the insurer.
13 Each insurer subject to this article shall issue a new card to each
14 insured whenever any of the information required to be included on such
15 card changes.
16 § 2. Section 4324 of the insurance law is amended by adding a new
17 subsection (f) to read as follows:
18 (f) Each health service, hospital service or medical expense indemnity
19 corporation subject to this article shall issue to each subscriber an
20 insurance identification card which shall include the following informa-
21 tion:

1 (1) the subscriber identification information, including the subscrib-
2 er's name and identification number;
3 (2) the name of the product the corporation uses to describe the bene-
4 fits covered under the contract;
5 (3) the amount of the subscriber's co-payment, if any; and
6 (4) the billing address and contact information, including a telephone
7 number, for the health service, hospital service or medical indemnity
8 corporation.
9 Each health service, hospital service or medical indemnity corporation
10 subject to this article shall issue a new card to each subscriber when-
11 ever any of the information required to be included on such card chang-
12 es.
13 § 3. Section 4408 of the public health law is amended by adding a new
14 subdivision 7 to read as follows:
15 7. Each health maintenance organization subject to this article shall
16 issue to each subscriber an identification card which shall include the
17 following information:
18 (a) the subscriber identification information, including the subscrib-
19 er's name and identification number;
20 (b) the name of the product the health maintenance organization uses
21 to describe the benefits covered under the contract;
22 (c) the amount of the subscriber's co-payment, if any; and
23 (d) the billing address and contact information, including a telephone
24 number, for the health maintenance organization.
25 Each health maintenance organization subject to this article shall
26 issue a new card to each subscriber whenever any of the information
27 required to be included on such card changes.
28 § 4. This act shall take effect on the first of January next succeed-
29 ing the date on which it shall have become a law and shall apply to any
30 contract issued, renewed, modified or altered on or after such date.
