        AN ACT to amend the public health law, in relation to the creation of an
          online standardized medical physical examination form
 
          The  People of the State of New York, represented in Senate and Assem-
        bly, do enact as follows:
 
     1    Section 1. The public health law is amended by adding a new section 25
     2  to read as follows:
     3    § 25. Standardized medical physical examination form. The commissioner
     4  shall establish an  online  standardized  medical  physical  examination
     5  form,  which  may be utilized by, but not limited to, schools, colleges,
     6  universities, camps for children and doctors' offices.
     7    § 2. This act shall take effect on the ninetieth day  after  it  shall
     8  have become a law.
