        AN ACT to amend the vehicle and traffic law, in relation to the issuance
          of distinctive plates bearing the words  "Cure  Parkinson's  Disease",
          and  to  amend  the state finance law, in relation to establishing the
          "Cure Parkinson's Disease Trust Fund"
 
          The People of the State of New York, represented in Senate and  Assem-
        bly, do enact as follows:
 
     1    Section  1.  The  vehicle  and  traffic law is amended by adding a new
     2  section 404-x to read as follows:
     3    § 404-x. Distinctive "Cure Parkinson's Disease" license  plates.    1.
     4  Any  person  residing  in  this  state  shall  upon  request be issued a
     5  distinctive license plate bearing the words "Cure Parkinson's  Disease".
     6  Application  for such license plate shall be filed with the commissioner

     7  in such form and detail as the commissioner shall prescribe.
     8    2. Any distinctive plate issued pursuant  to  this  section  shall  be
     9  issued in the same manner as other number plates upon the payment of the
    10  regular  registration fee prescribed by section four hundred one of this
    11  article provided, however, that an additional annual service  charge  of
    12  twenty-five  dollars  shall be charged for such plate. Twenty dollars of
    13  each such twenty-five dollar annual service charge shall be deposited to
    14  the credit of the cure Parkinson's disease trust fund established pursu-
    15  ant to section ninety-nine-u of the state finance law and shall be  used
    16  by  New York state university Parkinson's disease research hospitals for

    17  research and education programs approved by the commissioner of  health.
    18  Provided,  however  that  one year after this section shall take effect,

     1  funds in the amount of five thousand dollars, or so much thereof as  may
     2  be available, shall be allocated to the department to offset costs asso-
     3  ciated with the production of such license plates.
     4    § 2. Section 508 of the vehicle and traffic law is amended by adding a
     5  new subdivision 6 to read as follows:

     6    6.  The  commissioner  is  directed to develop license application and
     7  renewal forms which shall solicit a voluntary one dollar  donation  from
     8  persons  applying for or renewing a driver's license at the time of such
     9  application or renewal. Any moneys collected pursuant to  this  subdivi-
    10  sion  shall  be  deposited  in  the  cure Parkinson's disease trust fund
    11  established pursuant to section ninety-nine-u of the state finance law.
    12    § 3. The state finance law is amended by adding a new section 99-u  to
    13  read as follows:
    14    § 99-u. Cure Parkinson's disease trust fund. 1. There is hereby estab-
    15  lished  in the joint custody of the commissioner of taxation and finance
    16  and the comptroller, a special fund to be known as the "cure Parkinson's

    17  disease trust fund".
    18    2. Such fund shall consist of all revenues received  pursuant  to  the
    19  provisions of section four hundred four-x of the vehicle and traffic law
    20  and all other moneys appropriated, credited, or transferred thereto from
    21  any  other  fund  or  source  pursuant to law. Nothing contained in this
    22  section shall prevent the state from receiving grants, gifts or bequests
    23  for the purposes of the fund as defined in this section  and  depositing
    24  them into the fund according to law.
    25    3.  Monies  of the fund shall be expended only for Parkinson's disease
    26  research and education projects approved by the commissioner of  health,
    27  or  to  provide  grants to New York state university Parkinson's disease

    28  research hospitals for the purpose of researching and promoting  a  cure
    29  for Parkinson's disease.
    30    4.  Monies  shall be payable from the fund on the audit and warrant of
    31  the comptroller on vouchers approved and certified by  the  commissioner
    32  of health.
    33    5.  To the extent practicable, the commissioner of health shall ensure
    34  that all monies received during a fiscal year are expended prior to  the
    35  end of that fiscal year.
    36    § 4. This act shall take effect on the one hundred eightieth day after
    37  it  shall have become a law provided, however, that the addition, amend-
    38  ment and/or repeal of any rule or regulation necessary for the implemen-
    39  tation of this act on its effective date is authorized to  be  made  and
    40  completed on or before such date.
