        Introduced by Sens. FUNKE, MURPHY -- read twice and ordered printed, and
          when printed to be committed to the Committee on Labor
 
        AN  ACT  to  amend  the  workers'  compensation law, in relation to form
          reduction and streamlining
 
          The People of the State of New York, represented in Senate and  Assem-
        bly, do enact as follows:

     1    Section  1.  The  workers' compensation law is amended by adding a new
     2  section 140-cc to read as follows:
     3    § 140-cc. Form reduction and streamlining. The board shall develop and
     4  implement a form reduction strategy designed to improve,  modernize  and
     5  streamline  all  forms  used in conjunction with a workers' compensation
     6  claim from inception to closure. The board shall complete the  reduction
     7  and  streamlining of the C-4 series of medical forms and the implementa-
     8  tion of the medical portal by December thirty-first, two thousand seven-
     9  teen. The board shall complete the reduction  and  streamlining  of  all
    10  other  board  forms, as appropriate, by December thirty-first, two thou-
    11  sand eighteen.
    12    § 2. This act shall take effect immediately.
