Provides that the coroner, coroner's physician or medical examiner shall promptly report to the division of veterans' affairs, in a one page form developed and provided by such division of veterans' affairs, any death which appears to be a suicide by a person who, to the knowledge of the coroner, coroner's physician or medical examiner, is a veteran.

AN ACT to amend the county law and the executive law, in relation to the
reporting of a veteran's suicide
The People of the State of New York, represented in Senate and Assem-bly, do enact as follows:
1 Section 1. Section 677 of the county law is amended by adding a new
2 subdivision 10 to read as follows:
3 10. The coroner, coroner's physician or medical examiner shall prompt-
4 ly report to the division of veterans' affairs, in a one page form
5 developed and provided by such division, any death which appears to be a
6 suicide by a person who, to the knowledge of the coroner, coroner's
7 physician or medical examiner, is a veteran.
8 § 2. Section 353 of the executive law is amended by adding a new
9 subdivision 25 to read as follows:
10 25. To develop a one page form by which a coroner, coroner's physician
11 or medical examiner shall fill out to report to the division any death
12 which appears to be a suicide by a person who, to the knowledge of the
13 coroner, coroner's physician or medical examiner, is a veteran. This
14 form shall provide the address to which the form is to be mailed or
15 submitted electronically.
16 § 3. Within 60 days of the effective date of this act, the division of
17 veterans' affairs shall establish and implement the procedures and forms
18 necessary for the implementation of this act.
19 § 4. This act shall take effect immediately.
